A vasectomy just diverts sperm away from the ejaculate.
If the two of you want to have kids, the easiest thing to do would be artificial insemination. There’s no point in going in to reconnect everything when all you need is a sperm sample to accomplish what you need done.
I don’t know if 20 years with a vasectomy effects anything or not. Sperm seem to be ridiculously resilient in their regeneration … but adequate counts may be an issue at age 42 regardless.
Either way, you should see a reproductive specialist and be open to the idea of artificial insemination if they recommend it.
I am also 42. I had a vasectomy at age 21. I never had children, and planned it that way. A few years back I did a term paper on the subject. While it is true that I am no doctor, I can tell you that your chances are near zero.
Now for the "reality check"…..His chances were about 25% had he had the reversal done in the first five years. The chances go down from there. Are you aware that the surgery costs between $5000 and $10,000, takes three hours, and is very painful? Are you aware that ICF is almost always required and that costs about $17,000 per attempt? Are you aware that it usually takes 2-3 years to get pregnant after a vasectomy reversal? Are you aware that doctors are in the business of selling false hope? Are you aware that he will be 53 and have a teenager in the house?
Is there hope? Yes. Ask your doctors about "sperm aspiration". It is a lot less intrusive, cheaper, and has a lot better success rate (however IVF is still required).
