But if you can’t come to an agreement, talk to the sisters. It is not clear from your question whether the future husband intends to live there or whether his girlfriend is backing out of the lease. It sounds like you need another roommate to replace this one.

As for the other sick one, I’m not sure what to make of her story. What proof do you have that she is this ill and why isn’t she doing anything about it? Sounds really fishy to me. But if she is actually dying, she should go home and allow you to replace her with a roommate that will be around to pay the rent for the full year.
